Overview
A bunion, also known by its medical name hallux abductovalgus, is foot condition in which your big toe points toward your second toe, causing a bump or prominence to develop on the inside edge of your big toe and first metatarsal bone. Your first metatarsal bone is the long bone located directly behind your big toe, in your mid-foot. A bunion will cause your forefoot to appear wider because the base of your big toe now points away from your foot instead of pointing straight ahead.
Causes
Bunions have a number of causes, primarily genetics and bad choices in footwear. We inherit traits like flat feet, abnormal bone structure, and loose ligaments and tendons from our parents. When our feet are weakened by these traits and we stuff them into high heels or shoes which don?t support our feet correctly, the repeated stress on the front of the foot may contribute to the formation of a bunion. Other contributing factors are jobs that demand a lot of time standing, obesity, and sudden hormonal changes and weight gain, as in pregnancy. Unfortunately, bunions can lead to many other foot conditions as well. The joint behind the big toe carries much of your body weight and when the bunion makes it sore, you shift your weight onto other areas of the foot. That?s why we frequently see crossover toes, overlapping toes, hammer toes, corns, calluses, and ingrown toenails accompanying bunions. As pain in your foot increases, you?ll also reduce your activity, becoming more sedentary, which has its own quality-of-life issues.
Symptoms
The most common symptoms associated with this condition are pain on the side of the foot just behind the great toe. A red painful bump is usually present. Pain is usually brought on with walking or sports. Shoes don't cause bunions but will typically aggravate them. Stiff leather shoes or shoes with a tapered toe box are the prime offenders. This is why bunion pain is most common in women whose shoes have a pointed toe box. The bunion site will often be slightly swollen and red from the constant rubbing and irritation of a shoe. Occasionally, corns can develop between the 1st and 2nd toe from the pressure the toes rubbing against each other.
Diagnosis
Most patients are diagnosed to have bunions from clinical history and examination. However, in some cases, X-rays will be performed to determine the extent of damage to the joint. Furthermore, it will enable the treating doctor to decide on the best course of management of the patient.
Non Surgical Treatment
Conservative Treatment. Apply a commercial, nonmedicated bunion pad around the bony prominence. Wear shoes with a wide and deep toe box. If your bunion becomes inflamed and painful, apply ice packs several times a day to reduce swelling. Avoid high-heeled shoes over two inches tall. See your podiatric physician if pain persists. Orthotics. Shoe inserts may be useful in controlling foot function and may reduce symptoms and prevent worsening of the deformity. Padding & Taping. Often the first step in a treatment plan, padding the bunion minimizes pain and allows the patient to continue a normal, active life. Taping helps keep the foot in a normal position, thus reducing stress and pain. Medication. Anti-inflammatory drugs and cortisone injections are often prescribed to ease the acute pain and inflammations caused by joint deformities. Physical Therapy. Often used to provide relief of the inflammation and from bunion pain. Ultrasound therapy is a popular technique for treating bunions and their associated soft tissue involvement.
Surgical Treatment
Surgery takes place either under local or general anaesthetic and takes about one hour. After surgery you will have either a plaster cast or special dressing on the foot and you will be given a special walking shoe and crutches to use the first few days/weeks. Recovery usually takes approximately 6-8 weeks but swelling often lasts longer and it may take a few months before you are able to wear normal shoes again. Full recovery can take up to a year. Bunion surgery is successful in approximately 85% of cases, but it is vital not to go back to wearing ill-fitting shoes else the problem is likely to return.
Prevention
To help prevent bunions be sure your shoes don't cramp or irritate your toes. Choose shoes with a wide toe box - there should be space between the tip of your longest toe and the end of the shoe. Your shoes should conform to the shape of your feet without squeezing or pressing any part of your foot. Avoid pointy-toed shoes.
